Medical evidence

You need to submit supporting medical evidence of your disability or health condition in order to register with the Disability Inclusion Service. This page offers guidance on the types of medical evidence you might provide.

What evidence can I submit?

The documents that you submit should indicate the disabilities or health conditions you experience. Below are some examples of evidence we accept, but please note that this list is not exhaustive.

  • Documents from Disabled Students Allowance (DSA) – such as your DSA Needs Assessment, a completed DSA medical evidence form (PDF, opens in new tab), or your DSA2 letter 
  • A doctor’s letter 
  • A diagnostic report (such as for ADHD or autistic spectrum conditions) 
  • Evidence of a referral for an ADHD or autistic spectrum condition (ASC) assessment. We understand that it can take a while to get an autism or ADHD assessment, so a referral allows you to get support from our service 
  • A copy of your medical history that shows your name and disability/s (e.g. screenshots from the NHS app) 
  • A photo of a prescription/prescribed medication which you take for your disability showing your full name and the date prescribed (e.g. SSRIs, anti-seizure medications) 
  • An in-date sick note from a GP 
  • A hospital discharge letter
  • An occupational health report
  • Medical evidence provided by your Wellbeing Adviser or counsellor 
  • For diagnoses of Specific Learning Difficulties such as dyslexia, an Educational Psychologist assessment*  

*Please note: to claim support for a Specific Learning Difficulty (SpLD), you must submit a full copy of an Educational Psychologist assessment, or a full report by a Specialist Teacher who is AMBDA qualified. If you suspect you may have a SpLD but have not had either of the above assessments, you can apply for one through the University. Please contact us for further details.
